The Home Health Aide is an employee of Affinity Home Care Agency and is in the home due to illness or disability of a family member. The Home Health Aide is a member of a Health Team. The Home Care Nurse, with the approval of your physician, and you will plan in writing the types of duties the Home Health Aide will be expected to perform in your home.

Any changes in these plans must be done in writing and approved by the Home Care Visiting Nurse. While Under the care of a Home Health Aide, a Home Care Nurse or the therapist will be making visits every two weeks to supervise the Home Health Aide. You will be asked about the care given by the Home Health Aide and your plan of care will be reviewed.
