What is home care?

Home health care is a service to receive personalized care in the comfort of your home while maintaining the quality of life. There are two kinds of home care services you should know about SKILLED CARE and UN-SKILLED CARE

"Home care" is a simple phrase that encompasses a wide range of health and social services. These services are delivered at home to recovering, disabled, chronically or terminally ill persons in need of medical, nursing, social, or therapeutic treatment and/or assistance with the essential activities of daily living.

When is home care appropriate?

Generally, home care is appropriate whenever a person prefers to stay at home but needs ongoing care that cannot easily or effectively be provided solely by family and friends. More and more older people, electing to live independent, non-institutionalized lives, are receiving home care services as their physical capabilities diminish. Younger adults who are disabled or recuperating from acute illness are choosing home care whenever possible. . Adults and children diagnosed with terminal illness also are being cared for at home, receiving compassionate care and maintaining dignity at the end of life. As hospital stays decrease, increasing numbers of patients need highly skilled services when they return home. Other patients are able to avoid institutionalization altogether, receiving safe and effective care in the comfort of their own homes.

 

 

What is Skilled Care?

SKILLED CARE Services provide you with a Registered Nurse, Physical Therapist, Occupational Therapist's, Speech Therapist's, Certified Home Health Aides, and Medical Social Workers. These services are covered by most insurances. ALL Skilled home care companies require doctors orders in order for a Nurse or Therapist to be sent to your home. What is Unskilled Care?

UNSKILLED "Private Care Services" provide you with experienced home health care Caregiver's who are able to provide NON-SKILLED Services for example: Companionship, Bathing, Cleaning, Transportation, Prepared Meals, Medication Education and more. Why Home Care?

Home Care is very cost-effective option for receiving health care services. If you have MEDICARE, skilled home care services are Covered 100% by MEDICARE. However, if you need PRIVATE CARE, you will have to pay hourly. Returning to one's home and family can quicken improve the quality of life, for both patients and families or caregivers.

 

I'm in the hospital?

By law it's PATIENTS CHOICE to choose the home care agency you want in the comfort of your home. Many health care facilities have their own home care agency, however, that doesn't mean you have to use them. You get to choose who services you or loved one when you're in the hospital, rehab center and assisted living.The home health care agency you choose to work with should be notified by you (or family), doctor or case manager before you leave the hospital. To be eligible for Skilled Home Care.

  • Be under care of a physician
  • Need doctors orders faxed to us FAX LINE: 248.363.8652
  • Unable to leave the home unassisted
  • Require intermittent skilled care: Nursing, Therapy, Aides, Social Workers
  • Have a plan of care established by a physician
  • INSURANCE ACCEPTED: Medicare, Medicare Advantage, Blue Cross Blue Shield,  Cofinity,  AETNA, Auto Insurance

To be eligible for Un-Skilled Home Care
  • There's no need for a doctor to be involved
  • Schedule a consultation to meet with our CAREGIVER COORDINATOR
  • Doesn't have to be home bound
  • Long Term Care
  • BATHING, CLEANING, COMPANIONSHIP, MEAL PREPARATION
  • There's no need for a NURSE or THERAPIST
  • PAYMENT: Cash, Check, Online Payment (Visa or Mastercard)

I'm not happy with my current home care

If you're not happy with your current home care agency, you have the choice to switch services to another company at anytime. All you need to do is call our office and we will work with your physician to get a new doctors orders. On our first visit you will have to sign a TRANSFER OF SERVICES form. It's a very painless process and GUARANTEE we will have someone there within 48 hours.
