I'm in the Hospital.
By law you have the right to choose what home care you decide to work with. Our staff should be notified by you (or family), doctor or case manager before you leave the hospital. Please have your family member(s) or doctor contact us right away.
To be eligible for home care.
- Be under care of a physician *Need doctors orders faxed to us.
- Unable to leave the home unassisted
- Require intermittent skilled care: Nursing, Therapy, Aides, Social Workers
- Have a plan of care established by a physician
